Community Rehab Physical Therapy Centers ("we," "us") respects your privacy. This policy explains what we collect through this website, how we use it, and the choices you have. It covers the website only; the health information we create and keep as your healthcare provider is governed by HIPAA and our Notice of Privacy Practices (see below).
Information we collect
- Information you give us through forms. When you use our contact, insurance-verification, or physician-referral forms, we collect the details you enter — such as your name, phone number, email, preferred clinic, insurance, and the reason you're seeking care. Some of this may be health-related.
- Technical information. Like most websites, our host may log basic technical data (such as IP address and browser type) to keep the site secure and running. We do not run advertising or behavioral tracking on this site.
We ask you not to include detailed medical history in website forms. For anything sensitive, please call or fax the clinic.
How we use your information
- To respond to your message, schedule an appointment, or verify your insurance benefits.
- To coordinate a referral from your physician's office.
- To operate, secure, and improve the website.
We do not sell your personal information, and we do not use it for third-party advertising.
Cookies & embedded content
We use essential cookies needed to run the site, and we embed Google Maps to show our clinic locations. Loading a map shares some data (including your IP address) with Google, under Google's own policies. We do not run advertising or behavioral tracking. This site also loads fonts from Google Fonts; if you'd prefer, we can self-host these on request. You can review this notice anytime via the Cookie settings link in the footer.
Who we share information with
We share information only with service providers that help us operate, and only as needed:
- Email delivery (to route form submissions to our staff).
- Website hosting (to serve and secure the site).
- Google Maps / Google reviews (map display and showing our public review rating).
Where any provider handles health information on our behalf, we require a Business Associate Agreement as HIPAA requires. We may also disclose information if required by law.
Your health information (HIPAA)
As a healthcare provider, the medical information we create and maintain about you is protected health information under HIPAA. How we use and protect it, and your rights to it, are described in our Notice of Privacy Practices — contact us for a copy. The website forms are a convenience for reaching us; please limit the health details you enter online.
Data retention & security
We keep website inquiry information only as long as needed to respond and for our records, then dispose of it appropriately. The site is served over HTTPS. No method of transmission is perfectly secure, but we take reasonable steps to protect your information.
Your choices & rights
You may ask us what website information we hold about you, ask us to correct or delete it, and opt out of non-essential cookies. To make a request, contact us using the details below. Depending on where you live, you may have additional rights under your state or country's laws; we honor applicable rights.
Children
This website is not directed to children, and we don't knowingly collect information from children through it. We do provide pediatric therapy — those arrangements are made by a parent or guardian directly with the clinic.
Changes
We may update this policy and will revise the effective date above when we do.
